Referee Pay Report Forms


HSA Recreational Game and Pay Report Forms

All referees working Hancock Soccer Association Recreational matches are required to turn in a HSA Recreational Game Report for each match.  All required information on this form MUST be completed and legible.

Failure to complete these forms and getting required coach signatures will result in no pay.  These forms are your proof that you worked the assigned match, and are used to verify your attendance for pay purposes.

The other information required on these forms is used by HSA in determining problem areas that may need looked into.  Information on these reports is used for checking on unsporting behavior of coaches (such as running up match scores), determining possible imbalance of team skills, and seeing if certain teams have persistent unruly parents.

All CAUTIONS, SENDOFFS, and EJECTIONS are tracked by HSA for possible disciplinary actions.  Therefore, it is imperative ALL referees complete these forms entirely (ALL INFORMATION FILLED IN) and LEGIBLY.

Each referee must submit a HSA recreational game and pay report form for themselves individually.  Completed report forms should be dropped off at the Emory Adams Park HSA equipment shed prior to leaving the park at the end of your officiating day.  Do not "accumulate" these forms all season, drop them off at the shed upon leaving Emory Adams Park.  A mail slot labeled Referee Information is on the east side of the shed for referees to deposit their completed forms in.

BSSL Roster Verification Form

In addition to the HSA Recreational Game and Pay Report Form, each CENTER Referee MUST complete a team roster verification form for all matches between Hancock Soccer Association (HSA) and Black Swamp Soccer League (BSSL) teams.  Assistant Referees do not need to complete a form, only the Center Referee needs to turn one in.  If you are the ONLY Referee on the match, then you need to turn one in.  One verification form per match.

These verification forms MUST be turned in to the Findlay Certified USSF Assignor (use the Referee Information Drop Box located at the HSA Shed near the center of the Emory Adams Park complex) upon leaving Emory Adams Park.

Roster verification forms will be available at the Emory Adams Park fields HSA equipment shed.  You may also download and print copies of the report form using the link provided below.
