Recreational Referee Evaluation Form

Hancock Soccer Association Recreational coaches are encouraged to complete Referee Evaluation forms and turn them in after each match.  These evaluation forms are forwarded to the Findlay area Certified USSF Referee Assignor.  The information that coaches report on these forms helps the assignor determine who needs some mentoring or additional training, which referees are doing good jobs, etc.

A link is provided below to download and print these Referee Evaluation forms.

Turn in completed evaluation forms at the Hancock Soccer Association equipment shed at Emory Adams Park.  The shed is located in the center portion of the park.  There is a mail slot on the east side of the shed marked "Referee Information".  Deposit your completed forms in that mail slot.
