HSA Recreational Divisions Game Reports

Hancock Soccer Association requires game reports to be completed for all U7 through U19 games where Referees have been assigned to officiate.  When working as a three person crew (U12 and older), the Center Referee is responsible for completing the game report (AR's do not need to complete individual reports).

Referees will need to have game reports with them prior to each match.  HSA will attempt to have blank forms available at the Emory Adams Park HSA equipment shed.  It is suggested that Referees download and print blank copies to have in their equipment bags in case game reports are out of stock.

REPORT COMPLETION INSTRUCTIONS:

  1. Fill in Date, Time, Field, Game #, Age Div, and Gender in the spaces provided at the top of the form.
  2. Referees are required to check the goals to verify that they are properly anchored, safe, and secure prior to play.
    • This is required per USYSA, OYSAN, BSSL, and HSA Goal Safety Policies.
  3. Referees need to check in both teams prior to play.
    • For U12 through U19 teams:
      • Referees are required to check in both teams using the official BSSL team rosters prior to play.
      • Each head coach needs to sign the game report form to acknowledge that the Referee checked in their players using their respective team roster and that all players participating in the game are the players listed on their certified BSSL team roster.
    • For U7 through U10 teams:
      • HSA does not require coaches to have their team roster in their possesion at matches.
      • Player check in consists of making sure players are wearing proper equipment and that jewelry is removed.
      • Each head coach needs to sign the game report form to acknowledge that all players participating in the game are the players listed on their certified HSA team roster.
    • For all games, the coaches must sign the form before game play begins!
  4. The Referee must initial the box on the game report form to indicate that goals were checked and that U12 through U19 rosters were checked.
  5. Referee crew member names must be PRINTED CLEARLY in the spaces provided on the form.
  6. Home and Visitor team names and scores must be recorded in the spaces provided on the form.
    • Note: Scores are not used for league record rankings (league rankings are not permitted by USYSA and OYSAN for member Recreational Leagues), but they still need to be reported to HSA.
      • Scores are needed to help the league determine if certain teams are:
        • Running up the score on their opponents (poor sportsmanship).
        • Dominating most games and may need roster adjustments for the next season.
  7. All Cautions (Yellow Cards) must be recorded in the spaces provided on the form.
    • Make sure you record the player(s) name, team, and reason for the Yellow Card on the form.
  8. All Send-Offs (Red Cards) must be recorded in the spaces provided on the form.
    • Make sure you record the player(s) name, team, and reason for the Red Card on the form.
  9. All Misconduct incidents (non-cardable incidents) involving players, coaches, and/or spectators are to be reported using the Referee Comments section of the form.
    • Be sure to include details about the incident (ie: ejection, Referee badgering, Referee abuse, Referee assault, fighting, etc), including offenders names.
  10. Any Referee comments about something that needs to be brought to the attention of the league should be entered in the Referee Comments section of the form.
    • If additional room is needed for comments, write them on the reverse side of the form.
  11. After all game information is entered on the form, the Referee must sign the form (signature box is located at the bottom of the form).

The Referee must turn in the completed game reports at the HSA equipment shed located near the center path of Emory Adams Park prior to leaving the fields for the day (deposit them in the "Referee Information" mail slot that is on the east side of the shed).  Do not accumulate completed report forms for dropping off at a later date.  These game reports are needed ASAP by the league!  Game reports will be collected by HSA after each weekend of games (and on weekdays as needed).

Hancock Soccer Association will also have forms available for Referees at the Emory Adams Park HSA equipment shed.  They will be in a container that is mounted on the east side of the shed.


HSA Recreational Divisions
Yellow Cards, Red Cards, and Misconduct

Both Hancock Soccer Association and the Black Swamp Soccer League REQUIRE that Referees MUST report all Yellow Cards, Red Cards, and Game Misconduct.  Below are the procedures Referees are to use to report these incidents.

YELLOW CARDS:

Yellow Cards are to be reported using the spaces provided on the HSA Recreational Game Report.  For U12 or older matches, Hancock Soccer Association will forward the completed Yellow Card reports to the Black Swamp Soccer League for you.

RED CARDS:

Red Cards are to be reported using the spaces provided on the HSA Recreational Game Report.

In addition to completing the HSA game report, you MUST report the Red Cards to the HSA President AS SOON AS POSSIBLE following the end of the match.  Contact information can be found on the HSA website Contact Us page.  You will also need to complete an additional written report (USSF Misconduct Report) that needs to be sent to the HSA President (within 48 hours of the incident).  Contact the HSA President to find out how he/she prefers you to deliver these reports to him/her.  For U12 or older matches, the HSA President will forward the completed reports to the Black Swamp Soccer League for you.  If necessary, the HSA President will also forward the report to any other appropriate USSF and/or OYSAN authorities for you.

MISCONDUCT:

All Misconduct incidents (non-cardable incidents) involving players, coaches, and/or spectators are to be reported using the "Referee Comments" section on the HSA Recreational Game Report.

Referee Evaluation Report Form

The Findlay Certified USSF Referee Assignors would like each Center Referee to supply Referee evaluation forms to the coahes of both teams.  Explain to them that their input will be greatly appreciated.  Tell them that they can deposit their completed evaluation form in the "Referee Information" mail slot that is on the east side of the HSA equipment shed located near the center path of Emory Adams Park or they can mail it to the address listed on the bottom of the form.  A download link for this form is provided below.

How To Deal With Problem Parents

  • Remain calm.
  • Do not get into discussions or arguments with the sidelines.
  • Enlist the support of the coach.  Ask the coach to speak with the offending spectators and let the coach know that if the behavior continues, the game will not.  This will usually be enough to quiet most parents (other parents may understand the consequences and help with the problem spectator).
  • If you have asked the coach to deal with problem parents and the situation continues, ask the coach to have the spectator leave the area.  If the spectator refuses, tell the coach that if the spectator is not removed, the game will end.  Give the coach a reasonable amount of time (a few minutes) to deal with the situation.  Remember, you cannot dismiss a spectator directly but must work through the coach or a tournament or league official.
  • If the parent does not leave, you should feel free to end the game.  Include any misbehavior on the part of the spectators in your game report to the league so that this type of behavior can be disciplined and stopped.  Most leagues and state associations have methods for dealing with bad behavior, but this often requires a written report from the Referee.
  • As a very last resort, you may call the local police for assistance.  It is preferable to avoid this situation if possible.  Only do this if it is deemed absolutely necessary!
